How do I make an appointment?
All patients must complete new patient paperwork first. Our providers will review it before we can schedule your appointment. Please call our office to request the paperwork.
What if I need to cancel my appointment?
To cancel your appointment, please call our office at (812) 479-1511 or use MyChart. We kindly request at least 24 hours’ notice, or as much advance notice as possible.
If it’s your first time at our office, we want to get to know you and your personal health care needs. You’ll want to bring some items with you to make sure you get the care you need at each visit.
- Photo ID and Insurance
- Bring a list of all medications you’re currently taking.
- Write down any important information regarding your recent health history and information about your previous mental health care.
The more we know about your past health and health records, the easier it will be to learn how to help you.
How do I refill a prescription?
To refill your prescriptions, please contact your pharmacy or send a MyChart request 3–5 business days before you run out. If there are no refills left on your prescription, the pharmacy will call or fax the request directly to our office. Please allow 72 hours for your refill to be sent to your pharmacy.
